I am fully authorized to prescribe all medications, including controlled substances (e.g. benzodiazepines and stimulants). However, prescription/refill of controlled substances would be contingent on comprehensive assessment, including review of your past medical records. Unfortunately, I do not have the required set up to prescribe opioid medications in my practice.

I am Out-of-Network with all insurance plans except for specific number of slots for Mount Sinai UMR Top Tier and Aetna commercial plans. Remaining out of network allows me to maintain a focus on providing high quality of care. Many health insurance plans offer out-of-network benefits which will reimburse a substantial portion of your treatment costs. I will provide you with an itemized detailed superbill so that you may submit a claim to your insurance company. I will also do my best to answer any questions to help facilitate this process. The following bullets can be helpful in learning about your individual insurance plan’s benefits and help you determine any out-of-pocket expenses:
- Call the number on the back of your insurance card.
- Note the date and time of the call. Ask the representative his/her name and direct contact number.
- “Does my insurance plan include out of network benefits for outpatient behavioral/mental health? If so, what are the benefits?”
- “Do I need to obtain prior authorization to have these services covered? If so, how do I go about that?”
- “Does my plan have an annual out-of-pocket maximum (which is usually the sum of my deductible and co-insurance) that I am expected to meet before my benefits kick in? If so, what is the amount?”
- “When my benefits do kick in, how much will be covered and how much will I be responsible for”? (For example, a plan might have an annual out-of-pocket maximum of $1000. After I have spent this much, my plan will reimburse 70% of my bill.)”
- “Are there specific claim forms that I must submit and is there a time frame in which the claims must be sent in by? Where do I submit the claims?”
- “How can I apply for the network gap exception or out-of-network exception?“ (This option allows insurance to cover out-of-network services when no suitable in-network provider is available within a reasonable distance, time, or expertise.)
